What is General Care Management?
General Care Management is a Medicare program for Federally Qualified Health Centers (FQHCs) and Rural Health Centers (RHCs) that reimburses providers when the service requirements for one of the following have been met:
- Chronic Care Management
- Complex Chronic Care Management
- Behavioral Health Management
General Care Management Details
– In 2018, the Centers for Medicare & Medicaid (CMS) revised the way FQHCs and RHCs bill for care management services. With the new rule, FQHCs and RHCs will not be reimbursed for CPT Codes 99490, 99487, and G0507.
– NEW Reimbursement Code: G0511 was created for FQHCs and RHCs at a national rate of $62.28 for 2018.

Yes, General Care Management Code G0511 is listed as a CMS Designated Care Management Code which was approved for General Supervision. This means the clinical care team that is dedicated to your facility do not require direct physician supervision.
